You can unlock this trophy during the OG Loc’s mission Madd Dogg’s Rhymes, available after completing life’s a beach.
You must remember to save the game manually at your safehouse before starting the mission.
To unlock this trophy, you will need to stealth kill all the guards at Madd Dogg’s mansion.
Crouch down and lock on to the guard. After that, you have to sneak up on him and perform a knife strike as soon as CJ lifts the knife above his head.
You will need to make sure to go behind the guards without bumping into them.
If you alert the enemies, reload the autosave or manual save game.
You have to wait for this guard to turn around before entering the room.
You can kill these two guards later.
Wait in this dark spot for the next guard.
Now you can use the silenced pistol to kill the rest of the guards. Only headshots will count as stealth kills, so take your time to aim at their heads before shooting.

The trophy will pop up as soon as you leave the mansion. If the notification doesn’t appear, you will need to reload the last saved game.
